Clinic Policies
- We charge $55 to cancel appointments with less than 24 hours notice, and for all missed visits.
- If you or someone you know have experienced within the last 21 days; cough, shortness of breath, fever, chills, spontaneous loss of taste or smell, cough, sore throat, and/or muscle pain consider getting tested for COVID-19 and contact us to reschedule your appointment.
- Please be scent-free, since we treat people with allergies and asthma. For example: No perfume, cologne, lotions, wet nail polish, etc. and do not smoke for at least 3 hours before your appointment.
- Please wait outside in your car or in the lobby. Do not knock on the door. We will let you in at your scheduled appointment time.
- Accompanying visitors will not be allowed, except if they are a caregiver or medical decision-maker for the patient.
- Turn off all electronic devices before entering the clinic and speak softly when in the office so not to disturb others.
Planning for your visit
- Make sure you have eaten something prior to your appointment.
- Plan to wear comfortable clothing that can be easily and comfortably pushed up to your elbows & knees.
- It is a good idea to allow at least 1½ hours for your clinic visit.
If you plan on using health insurance:
We are in network with United Healthcare, Optum, and Cigna as well as out of network with select insurance companies. We do not work with plans that are managed by American Specialty Health (ASH). Complete & submit the Insurance Verification form below. We will be in contact with your insurance information within 2 to 3 business days. If we are unable to verify your insurance benefits then payment in full is due at the time of service.
